MADELINE A. SIVAK, age 57, a resident of Chagrin Falls, OH, died November 13, 2014 at O'Neill Healthcare in Bay Village following a long illness. She was preceded in death by her parents, Dr. Michael V. and Madeline Sivak (nee Mentz) of Erie, PA.
Madeline was born August 30, 1957 in Erie and graduated from Mercyhurst Preparatory School in 1975. She moved to Cleveland, OH to attend college, earning a BA from John Carroll University in 1980. She received an RN diploma from Huron Road Hospital School of Nursing in 1985 and a graduate certificate in nonprofit management from Case Western Reserve University in 1994. Madeline also received certification in psychiatric mental health nursing from the American Nurse Association.
She was the founder and executive director of Michael's House for Grieving Children in Chagrin Falls, OH. Michael's House opened in 1995 and was the first grief support center for children in Northeast Ohio.
Madeline was a member of numerous professional, civic and charitable organizations throughout her life including the Junior League of Cleveland. She was the recipient of the Mercyhurst Prep distinguished alumni award for service in 2001, as well as the recipient of the Greater Cleveland Woman of Achievement and Cleveland Magazine's Most Interest People Awards, among others.
Surviving are her siblings, Dr. Michael Sivak (Donna) of Morgantown, WV, Dr. Edward Sivak (Barbara) of Westlake, OH, Mary Caputo (Steven) of Skillman, NJ, John Sivak (Mary) of Springfield, IL and Dr. Joseph Sivak (Lorraine) of Duluth, MN, 13 nieces and nephews and 14 great nieces and nephews.
A Mass of Christian Burial will be celebrated at St. Ladislas Catholic Church (2345 Bassett Rd., Westlake, OH 44145) on Wednesday, November 19, at 11 A.M.. Friends may call at CHAMBERS FUNERAL HOME of NORTH OLMSTED, 29150 LORAIN RD. AT STEARNS RD., TUESDAY 2-4 & 6-8 P.M.
Interment will take place at Trinity Cemetery, 2971 West Lake Rd., Erie, PA 16505 on Thursday, November 20.
The family suggests donations be made in Madeline's name to The Alzheimer's Association of Cleveland, Cleveland Catholic Charities, or Hospice of the Western Reserve.
