MATTHEW "MATEJ" POLIAK, age 73, passed away peacefully, surrounded by his wife and daughters, on Tuesday, July 13, 2021.

Beloved husband of 49 years to Ludmila (nee Kucera); loving father of Donna Ross (Shawn) and Andrea Babjack (John); dear grandfather of Matthew, Anderson, Dylan, and Clara; son of the late Helen and Rudolf Poliak; brother of Yarmila Takac (Mikulas) and Rudy Poliak (Karen); son-in-law of the late Anna and Alojz Kucera; brother-in-law of the late Alojz Kucera (Marta), the late Eva Kucera, Anton Kucera (Gabriela, deceased), and the late Josef Kucera (Tereza, deceased); uncle to many; loved by all.

Born in Slovakia, he came to the United States in 1968, working for years at PPG Industries.  A hockey, soccer, and history enthusiast throughout his life, he was most passionate about and completely devoted to his wife, Ludmila, his daughters Donna and Andrea, and his family.

Family and friends may call on Friday, July 16 ^th , 2021, from 4-7pm, at CHAMBERS FUNERAL HOME of Cleveland, 4420 Rocky River Drive at Puritas.  Funeral Mass will be on Saturday, July 17 ^th , 2021, 10am, at BLESSED TRINITY (formerly Ascension), 14040 Puritas Avenue in Cleveland. PLEASE MEET AT CHURCH. Final resting place, inurnment, at HOLY CROSS CEMETERY in Brook Park, Ohio.  Masks are requested to be worn.
